NOTICE – Important Health Coverage Documents

In compliance with the Affordable Care Act (ACA), GCSD#1 employees’ Form 1095 for the 2025 tax year is available to employees upon request. This form provides information about the health insurance coverage offered to employees of Goshen County School District #1.

Requesting a copy of Form 1095:
Employees can request their Form 1095 through one of the following methods:
Email: Send a request to Human Resources staff at hlara@goshen1.org or nroth@goshen1.org
Phone: Call GCSD#1 Human Resources at 307.532.2171, to request a copy.
Mail: Send a written request to Goshen County School District #1 Human Resources, 626 West 25^th Avenue, Torrington, WY  82240.

Once a request is received, we will provide the Form 1095 within 30 days. This notice will remain posted through October 15, 2026.
